Welcome! Here are some forms that you may need! Please note that the "Preschool Medical Record Form" must be completed and returned for each preschooler. If you have a child with asthma, please complete the asthma action plan and medication form, as well as the authorization for release. If you have a child with food allergies, please complete the food allergy plan, bus allergy form, authorization for release, medication form, and food allergy plan of care form.

If your child is absent, please call and leave a message for me with the reason your child is absent so that I do not call you to notify you of your child’s absence.
